Katharine House Hospice is a registered charity with an 8-bedded In-Patient unit (IPU), providing specialist palliative care provision to adult patients, their carers and friends.  We are seeking a highly motivated and resilient individual to lead our patient admission and discharge process within the IPU. This is a temporary role for 12 months.

You will be the point of contact for all Inpatient referrals, responsible for co-ordinating patient flow, from admission to discharge within the IPU. You will work within a multidisciplinary team fostering positive relationships with our Therapies and Hospice at Home teams. You will be a main point of support for patients, their families, carers and colleagues.

You will be confident in taking the lead at multidisciplinary team meetings, engaging in discharge discussions and holding the medical team accountable for their role in projecting discharge dates. Outside of the organisation, you will foster close relationships with key referers and promote the Hospice to help widen our reach to more patients.

The successful applicant must be flexible and available to work all shifts across 7 days. Experience of working in palliative medicine is desirable but not essential. An enhanced DBS check is also required.
