We provide free care for people with incurable illnesses – not just cancer – including heart failure and dementia. We also support families, carers, and loved ones, as well as healthcare professionals.
Our resources can help you:
- Meet QOF (Quality and Outcomes Framework) requirements
- Maintain GSF (Gold Standards Framework) registers
- Explain hospice care and palliative support to patients and families
- Guide people through diagnosis, treatment and care planning
- Promote wellbeing and manage symptoms effectively
- Share details of local services and referral pathways
