Hospice at Home Team

Our team of healthcare assistants provide non-medical, practical hands-on care for people in the comfort of their own homes.

Get support

We provide personalised care and support in your own home, helping you receive the care you need in the place where you feel most comfortable. We also offer support for families and carers.

Families are sometimes worried that accepting our help means we will take over and they won’t be involved. We work with patients and families to understand what matters most to them and provide support in a way that suits their needs.

All of our care is free.

 

How we can help

End of life care at home

Support during the final stages of life, usually the last 1-2 weeks. We can provide up to three visits a day to support both the patient and those caring for them. We review needs regularly to make sure care remains right for each person.

Crisis care

Short-term support when a person’s condition or circumstances change suddenly and urgent help is needed. We aim to respond on the same day of referral and can provide care at home for up to two weeks while longer-term support is arranged.

Respite for carers

Day and night sits give family members and carers time to rest, attend appointments or take a break, while their loved one continues to receive care and support at home. Subject to availability.

Getting home

Helping people return home from hospital, the hospice or another care setting. We can provide short-term support for up to two weeks while longer-term care is arranged.

What to expect

Hospice at Home isn’t a long-term care service. Support is usually provided for up to two weeks while longer-term arrangements are put in place.

After a referral is received, we’ll contact you to arrange an access visit. During this visit, we’ll discuss your needs, explain how Hospice at Home works and how we may be able to support you.

Our Coordinators will stay in regular contact to discuss availability and arrange visits.

We work closely with district nurses and our Palliative Care Nurse Specialists to make sure you receive the support you need. Any nursing care that requires a registered nurse will be provided by the appropriate healthcare team.

Our support may include:

  • personal care such as washing and dressing
  • help with toileting and continence
  • repositioning for pressure area care and comfort
  • support with basic household tasks
  • respite care day or night sits
  • social and emotional support
  • gentle touch massage (M Technique).

How to access Hospice at Home

You'll need a referral from a healthcare professional such as a district nurse, community nurse or GP.

If you have any questions about Hospice at Home, our team is available every day from 8 am to 9 pm.

Our Advice Line is also available 24 hours a day, 365 days a year.

Advocacy

Sometimes information about care and treatment can feel difficult to understand.

If you need support to make decisions, understand your rights or have your voice heard, you can ask for help from an independent advocate.

Advocacy is free and available to anyone who needs support.
