What to expect
Hospice at Home isn’t a long-term care service. Support is usually provided for up to two weeks while longer-term arrangements are put in place.
After a referral is received, we’ll contact you to arrange an access visit. During this visit, we’ll discuss your needs, explain how Hospice at Home works and how we may be able to support you.
Our Coordinators will stay in regular contact to discuss availability and arrange visits.
We work closely with district nurses and our Palliative Care Nurse Specialists to make sure you receive the support you need. Any nursing care that requires a registered nurse will be provided by the appropriate healthcare team.
Our support may include:
- personal care such as washing and dressing
- help with toileting and continence
- repositioning for pressure area care and comfort
- support with basic household tasks
- respite care day or night sits
- social and emotional support
- gentle touch massage (M Technique).
